Accidents result in an enormous number of injuries and fatalities -- with high economic costs. These accidents range from mishaps in the home, to those in the medical environment, aviation, and military operations.
Often, the cause of accidents lies in how people interact with products, systems, and their environment. Well documented examples include pilot errors, driver fatigue, and medical team interactions.
Aptima views each human-system interaction as an opportunity to improve safety. We use approaches such as organizational design to create robust teams and processes, training to improve operator competence, and human-centered product design. The outcome is improved fit between the operator and the environment.
Our work includes efforts such as:
- How drift in procedures can contribute to shuttle accidents
- How training helmsman can reduce injuries in high speed boat operation
- How information flow can improve patient safety
- How play spaces can be used to teach parents about injury prevention
Examples of our work:
Safety Modeling for NASA
Modeled how practices and procedures "drift" over time and jeopardize safety, such as in the Shuttle accidents <more>

Naval Special Warfare Safety Training
To reduce injury risk, developed a comprehensive PC-based instructional system for helmsman skills <more>

Perioperative Patient Safety
Analyzed perioperative information transfer to identify vulnerable areas; designed interventions to improve info flow for patient safety

Simulation-based Child Safety Training for Parents
Within a museum play space, profiled possible at-home dangers; parents observed potential risk behaviors of their own children.
